Remote Patient Management
Using a piece of equipment called “the Communicator,” the LATITUDE system collects and transmits specific device data remotely from the patient's home. You, as a clinician, set the interrogation schedule for both routine follow-ups and remote monitoring between follow-ups, according to individual clinic, physician or patient's needs. The Communicator:
- Plugs into a standard phone jack and power outlet
- Is designed to remain plugged in continuously after initial set-up
- Fits easily on a nightstand or end table
There are two types of Communicators available.
Wireless Communicator – Automatically collects information from the implanted device at your predetermined times, without any patient initiation. The LATITUDE Communicator then transmits the information via the phone line to a secure server that you and your clinic staff can access via the LATITUDE website with a username and password.
Wanded Communicator – Collects the same information, except patients must actively participate in the data collection process. At the scheduled times you set, a light on the LATITUDE Communicator prompts the patient via the Action Button to place the wand over their implanted device. The Communicator will then walk the patient through the appropriate steps to send the device data.
Data Management
When you enroll your patients in the LATITUDE system you maintain control of the device interrogation schedule choosing when the checks are to occur. You set two schedules, one for routine follow-up and one for monitoring between follow-ups, which can be done as frequently as daily or weekly depending on the system you are using. Through the website, you also control off-cycle data transmissions, in the event you need a patient to send data other than during routine follow-up or monitoring check.

Types of remote transmissions:
- Scheduled remote device follow-ups – The information collected by the Communicator is similar to that of an in-office device check (without performing any threshold testing), including a real-time electrogram.
- Remote monitoring – The secure website allows you to select the remote monitoring schedule and the alerts that LATITUDE will monitor— both during and between routine follow-ups. If an alert is detected, the clinic is notified. For example, LATITUDE can notify the physician if the battery power of the implanted device is detected with an elective replacement indicator (ERI).
- Patient-initiated interrogations – The Communicator also has the ability to send off-cycle data transmissions, in the event you need a patient to send device data other than during scheduled follow-up or remote monitoring check. This is controlled through the secure website.
Additional Patient Data Provide by LATITUDE
Heart Failure Management tools – The LATITUDE system includes multiple tools to remotely monitor your heart failure patients, including but not limited to patient symptom questions, heart rate variability, activity log as well as optional external Bluetooth-enabled weight scale and blood pressure monitor*. The weight scale and blood pressure monitor readings can be sent daily and there is also an alert associated with changes in weight.
Streamlined Communication – LATITUDE's Custom Select architecture allows both the device- and health-following physicians to individually select follow-up and monitoring schedules based on their practice needs.
